How to register for classes:

1. Complete my Class Registration Form below

2.Submit payment below after completing the registration form in order to secure your spot (pay online via credit card below at the 6-Week or 8-Week class buttons)

3. Submit shot records to me to include rabies and distemper (scan and email, or email photos of shot records)

All dogs must have current shots to include Canine Distemper and Rabies. A copy of shot records will be required to be provided.

PAYMENT IS DUE AT TIME OF ENROLLMENT OF CLASSES TO SECURE YOUR SPOT IN THE CLASS AND TO AVOID SLOTS BEING TAKING AND THEN NOT BEING USED AT THE LAST MINUTE!
